Policy Structure:

• All products listed above are available as standalone coverages or can be combined with other products for a modular policy
• Up to $5M available in primary limits
• Minimum Premium: $1,500
• Deductible: $1,000

Submission Requirements:

• ACORD Commercial Insurance application and General Liability application with a complete description of operations/exposures (signed & dated by the insured when bound)
• Supplemental application(s) as appropriate (signed & dated by the insured when bound)
• Five-year currently valued company loss runs, with a description for losses over $10,000
